個人番号カード

Once you have received your “My Number” Notification Card, you can apply for an Individual Number Card if you wish to do so. The Individual Number Card is a plastic card which contains an IC chip, the bearer’s name, address, date of birth, gender, Individual Number, and has an ID photograph. The card can be used as an identification card as well as to receive a wide range of services including municipality services and electronic applications using an electronic certificate such as e-Tax, or obtaining copies of your resident register at a convenience store.

How to apply: Apply by post using the application form attached to the bottom of your Notification Card. Paste an ID photo to the form, stamp with your seal, and send in the enclosed envelope. Alternatively, apply electronically by accessing the website. In either case, it takes about 2 months from application to receive the postcard informing you to pick up the card from your local city hall office.
